Answer the following questions to determine your tax basis in shares of Verizon Communications. If you acquired Bell Atlantic, Contel, GTE or NYNEX shares on more than one occasion, your tax basis must be determined separately for the shares involved in each acquisition.

To determine your basis in Idearc or FairPoint shares, please use the Cost Basis Worksheet. If your shares of MCI, Inc were acquired by Verizon on January 6, 2006, please use the Cost Basis Worksheet to determine your Verizon basis.

Note: The tax basis for Bell Atlantic and NYNEX stock acquired in the AT&T divestiture need to be calculated separately for each company. If you acquired Verizon Communications stock starting April 1, 2008, your current tax basis is the same as when you bought the stock.

When did you acquire the stock? (i.e. 01/01/1900)

Note: If you acquired Bell Atlantic or NYNEX stock in the AT&T divestiture please select December 31, 1983 as your acquisition date.
